Responsibilities:
The Manager - Clinical Operations is a qualified Registered Nurse and is an employee of the Agency.
The Manager - Clinical Operations participates in all activities related to the professional services provided, including establishing criteria on qualifications and assignment of clinical personnel.
As a Manager - Clinical Operations you will:
Monitor to ensure coordination and follow-through is complete for all referrals to the hospice program
Assist in the coordination and assignment of all newly admitted hospice patients to the appropriate team and/or disciplines
Work collaboratively to ensure all admission documentation is completed within 24-hour period and dispersed among team members
Ensure communication of all patient admissions and changes in Plan of Care is appropriately relayed to each member of the Interdisciplinary Team and Primary Care Physician
Serves as member of IDT committee and participates in all IDT meetings in which admissions, certifications, recertification’s and medical management of the patients are discussed
Consults with the Hospice Medical Director in matters relating to patient care services
Participating in routine visits and admissions of patients in times of high census or low staff availability
Selects and maintains a qualified, well-organized staff to provide care for the needs of the patients
Is accountable and supervises the IDT team i.

RN's, LPN's, Medical Social Workers, Chaplin’s, and Hospice Aides
Establishes staffing patterns, which reflect the quality and quantity of various professional personnel necessary to plan, provide, and supervise the care rendered to patients and families
Establishes methods for coordination of care by all disciplines
Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Qualifications:
Required Knowledge, Skills and Experience
Is currently a registered nurse licensed in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
Two years or more of supervisory and/or administrative experience managing clinical teams
5 or more years of nursing experience
Must maintain a valid driver’s license and good driving record
Demonstrates a broad base knowledge of community health nursing and competent clinical skills
Ability to be available in person or telephonically to assist with after-hours administrative on call responsibilities
Proficient Computer skills including, but not limited to, use of Word, Excel, report software such as HomeCare HomeBase, and Outlook
Excellent verbal and written communication skills with demonstrated leadership ability
Excellent organizational skills
Ability to travel (up to 25% or as needed)
Preferred Knowledge, Skills and Experience
Certified Hospice and Palliative Nursing
Previous experience with Electronic Medical Records
Hospice experience
Previous experience in handling new hospice referrals from first contact through admission
A working knowledge of Community Health Accreditation Program (CHAP) on Accreditation, Quality Assurance, and Hospice Knowledge
